I'm working on a couple of these at the moment in California. The fee has typically ranged based on the client and the quality of the fit and finish of the facility.
For example, one of my clients builds very high end "resort-styled" independent and assisted living facilities where another one concentrates on tax credit affordable senior housing. It is difficult to generalize since there are so many variables, but assuming a full service contract structure, I have seen anywhere between 4% on the low end to 12% on the high end.

I've been asking a few friends here in Wisconsin and they are telling me 3% to 7%.
