actually for cover letters (which typically don't fall under formal rather conventional) it is not appropriate to use a title unless the person is a doctor.
On the other hand, if you know someone is married, you may want to consider using "Mrs."
I take positive note of the people who use "Mrs." to address correspondence to me. It shows that they are paying attention and know a little bit about me.

This may seem extremely minute...
but when addressing a cover letter to a woman and it is unknown if she is married or not, should you use ms. or mrs.

might want to go with Ms. we toss those letters/resumes first.
