does anyone have a good rule of thumb (either $/sf or percentage of overall budget) for design fees (concept, interiors, creative) for restaurant design? any thoughts would be appreciated.

You did say ANY thought. This one is mine*. Just as valid as the next guy's.
Fees typically reflect the scope of work, the location of the project and the budget.
I once designed a small restaurant for no fee and got to eat there free for a year. Northern Italian, penne con pomodoro and fresh Napoleons, I was very sad when that year was over.

At my family's restaurant we didn't pay anything for the design. They just made money by taking a cut of the materials and furnishings.
