I would consider Olson Kundig in Seattle a design-build firm, doing some great work.
After reading their book of recent works, I got the impression that Miller Hull Partnership (Seattle & San Diego) had a design-build component to the firm, although it seems hard to firmly put them in that category (or likewise, to not put them into it).
If you're interested in the Great White North, there seem to be quite a few in Nova Scotia. Check out Susan Fitzgerald's work.
