Cornell says that
"Applicant’s name, address, date, and year must be clearly marked on the cover (and on any detachable contents). The contents of a submitted portfolio should measure no more than 8-1/2 by 11 inches in size."
What do you think the "date" and "year" means? my birthday? or the submission date of the portfolio?
I think the year means the semester that you are applying to begin your studies (i.e. Fall 2012). Not sure why they would want any other date. To be safe, you should call or email their admissions committee, no harm in asking them!
