From what I remember, the application is 2 parts. The first part is your money, basic information. The second part had the mailling labels and the most substantial part of your application.
They are basic mailing labels (no number or anything). You just self address them and they use them to mail back information to you: acceptance letter, etc. You can just buy some and mail them with your application if they forgot to put them in your application.
i made my own labels... as they ask to "type" them. and i don't know where i could even start looking for one... so i liberated a sheet of labels from work and printed a set... it is... 8 labels total.
