How do you think something like was attached to the wall previously? - Some sleeves have screw holes, while some don't.  There are no signs that show anything was screwed from the side.

My guess:

There is a dowel/post that is set in the wall, screwed to blocking/stud and then GWB / finish around. Then the piece you have slides over and a set screw or epoxy or gummy bears holds it in place. Looks like someone just took a reciprocal saw to them and cut through the posts / screws.  The epoxy/set screws/gummy bears came loose during the sawing which is why some have them and some don't.
