Yes... Most of the time there's something wrong because of problems with communication.
I've sometimes thought that a good tool would be like a large easy-to-use blackboard where everybody's input would be seen at the same time by all - something that is marked up before/during/after meetings. A big thing would be a low threshold for using it, regarding necessary skills - now many databases and shared models can be a bit tricky for all of the possible users.
Another thing that could facilitate good and frequent use would be a sliding scale of specificity for the tool - many things are left unsaid/unmarked in meetings because they are guesses or somehow hard to define in "memospeak" - you just withold some information because it is hard to formulate it so that it would fit into the matrix of an official memo or the minutes of a meeting. If a tool would allow some "woolly" ways of transcribing ideas and problems, more information would be disseminated than through the usual channels that require exactness and crisp language. Of course - woolly things can be understood in many ways, and there should be discussion and specifications then afterwards, but to get the ball rolling would be of the highest importance.

Isn't this being done through the use of BIM models and shared databases?
It's something diffrent. The basic task of that tool is to improve communication between designers.
